That T. rex was published in Germany in Kosmos' "Bunter Kinder-Kosmos Saurier - Tiere der Urzeit" von 1986. The original book however was published in Italy, titled "Guarda e Scopri i Rettili della Preistoria". A lot of artists were involved, so I can't tell who made the actual T. rex design.
